The Threaded Fasteners Charitable Foundation Non-Profit Grant Committee has selected Victory Health Partners to receive a $1,000 grant for their program.

Victory Health Partners increases access to affordable and quality healthcare for low to moderate income, uninsured adults.

Through Victory’s Healthy Living Program and Prescription Food boxes health promotion and disease prevention within the lives of patients participating within the programs will increase.

• As the patients who participate in the program struggle with chronic disease such as heart disease, diabetes, and food insecurity, the outcomes will reflect improvements such as blood pressure and cholesterol numbers, BMI, lowering of A1c for diabetes, mental health wellness, and education to integrate 
  healthy living into everyday life.
• Patients who participate within the program receive an initial screening at their first appointment, at a 6-month appointment and at a year. Their labs are taken along with meeting with the doctor for further evaluation and education. Their wellness data is being tracked to report on their outcomes
  throughout their first year within the program.
